Gift Information

  • AMOUNT PER PAYCHECK – enter your $ amount per paycheck (i.e. $10 per month is $5 per paycheck)
  • AREA OR FUND NAME – NWPB-Radio or NWPB-TV
  • INDEFINITE – check this box for an ongoing donation
  • UNTIL THIS AMOUNT IS REACHED – when you enter an amount here, your donation will stop when this amount is reached

 

Total Deduction and Donor Signature

  • Enter the total amount per paycheck and the date you want the deduction to start. Then sign and date

  • GIFTS DESIGNATED TO – NWPB-Radio or NWPB-TV
  • AMOUNT PER MONTH – enter the amount you want to donate each month
  • UNTIL THIS AMOUNT IS REACHED – leave this box blank for an ongoing contribution or put in the ending amount if you want the donation to stop
  • INDICATE ON WHICH DAY(S) OF MONTH GIFT IS TO BE DEDUCTED FROM YOUR ACCOUNT – select the 1st or 15th
